Usage
Osteoporosis is a multi-factorial disease with dimensions of genetics, endocrine function, exercise and nutritional considerations. Of particular considerations are Calcium (Ca) status, Vitamin D, fluoride, magnesium and other trace elements. Several trace elements, particularly copper (Cu), manganese (Mn) and zinc (Zn), are essential in bone metabolism as cofactors for specific enzymes. Clinical studies demonstrated the efficacy of Ca, Cu, Mn and Zn supplementation on spinal bone mineral density in postmenopausal women. Studies also demonstrated the necessity of trace elements for optimal bone matrix development and bone density sustenance.
Among the trace elements in bone and hair, significant differences were found in the contents of zinc, copper and manganese between normal subjects and osteoporotic patients.
It is proposed that more attention should be given to dietary trace elements, especially copper, in the aetiology of post-menopausal osteoporosis. Osteoporotic lesions attributable to copper deficiency have been described in both man and animals and the hypothesis that a mild dietary copper deficiency may be implicated in the onset and progression of osteoporosis is also consistent with the epidemiological evidence.
Magnesium is an important element in health and in disease. Its metabolic importance is well known. Magnesium regulates active calcium transport. As a result, there has been a growing interest in the role of magnesium (Mg) in bone metabolism. A group of menopausal women were given magnesium hydroxide to assess the effects of magnesium on bone density. At the end of the 2-year study, magnesium therapy appears to have prevented fractures and resulted in a significant increase in bone density.
Interest in vitamin D treatment for osteoporosis has recently been revived because of the focus in various parts of the world on the elderly population, which is predominantly vitamin D deficient, in addition to postmenopausal osteoporosis due to estrogen withdrawal, which has been the central theme of osteoporosis research for many years.
Vitamin D supplementation improves calcium absorption and decreases bone loss. Hip fractures and other nonvertebral fractures may be prevented by vitamin D administration.
Vitamin D deficiency is a major unrecognized health problem. Not only does it cause rickets in children, osteomalacia and osteoporosis in adults, but may have long lasting effects. Chronic vitamin D deficiency may have serious adverse consequences, including increased risk of hypertension, multiple sclerosis, cancers of the colon, prostate, breast, and ovary, and type 1 diabetes.
Vitamin C is a required cofactor in the hydroxylations of lysine and proline necessary for collagen formation. It is essential for the forming of connective tissue and collagen.
Serum concentrations of vitamin K are reportedly reduced in older individuals and persons with osteoporotic fracture. Clinical trials in humans point to a beneficial effect of additional intakes of vitamin K in bone health.
High intake of antioxidant micronutrients, especially vitamin C, may reduce the risk of cartilage loss and disease progression in people with osteoarthritis.
